Band & Color Guard Registration 2026–2027

This helps us update our database (as well as the system used for important emails and schedules from the music directors), and you will learn about AV Music volunteer needs and how you can help. 

✔️ Complete these forms (2 Compulsory Paper Forms + 2 Optinal Paper Forms):

• (required online) Band Handbook

PARENTS and STUDENTS acknowledge that you have read the handbook as part of the Online Registration Google form.

• (required online) MB Uniform Contract /  CG Uniform Contract 

Acknowledge as part of the Online Registration Google form.

• Form 1 (printout required) Local Field Trip form
  Marching Band and Color Guard members only: print, fill in the form, sign, and turn in at Registration or to Band Office
  Note: The field trip form for the overnight trip to the Bands of America competition (10.23.26 to 10.25.26) will be processed during the school year.

• Form 2 (printout required) PUSD Emergency Medical Information form 

*ALL Marching Band, Color Guard, Concert Band, and Jazz Band students must turn in an EMI form. Marching Band and Color Guard students must turn in their EMI form at the BEGINNING OF BAND CAMP - SATURDAY AUGUST 3. Band students who are not in Marching Band or Color Guard may turn in their EMI forms to the Band Office when school is in session.

• Optional Form 3 (if needed) *Contract to Carry Life-Sustaining Medication 

For those who need to carry medication and/or equipment, please fill out this form and have it signed by your medical care provider.

• Optional Form 4 (if needed) *Administration of Medication Consent Form 

If you would like your student to receive medication at football games and competitions, please have this form filled out by your medical care provider and turn it into the Band Office. You will be contacted before the first marching band event regarding the medication.

• Form 5 (Online) Please also fill out Food Allergies & Dietary Restriction form (submit online only) for Snack Team.

✔️ Return signed forms at in person registration, outside of the band room, at one of these times: 

• Registration Day

  • Saturday August 1st, 7:00 p.m. to 8:00 p.m.  (*Note: New Member Information Meeting before Registration, 6pm-7pm), Band Room.

  • Monday August 3rd, 8:30 a.m. to 9:30 a.m., Band Room

• Make-up Registration Day

  • Saturday August 8th, 8:30 a.m. to 9:30 a.m., Band Room

[After Aug. 8th] Please bring donation checks and signed forms to the Band Room:

• Checks go into the wooden box (accessed by AFM treasurer - treasurer@amadormusic.org)

• Forms go into the metal bin (accessed by AFM registrar - registrar@amadormusic.org)

REQUESTED AMOUNTS (2026–2027)

While students may participate regardless of ability to donate, the annual budget is developed on the premise that all families will contribute as able. All donations are greatly appreciated, even if you cannot give the full amount. Please contact the AFM treasurer (treasurer@amadormusic.org) with any questions.

• Marching Band & Color Guard (not in Wind Ensemble): $1,600

• Marching Band & Color Guard with Wind Ensemble: $1,720

• Wind Ensemble I/II only: $120

• Concert Band (Purple/Gold) only: $60

• Jazz A, Jazz B, Jazz Workshop: $150

• Band Seniors (includes Marching Band, Color Guard, Concert & Jazz Bands): $80

Examples:

Families with a student in Freshman Concert Band and Marching Band are requested to contribute a total of $1,600.

Families with a student in Symphonic Purple Band and Marching Band are requested to contribute a total of $1,660.

Families with a non-Senior student in Wind Ensemble, Marching Band, and Jazz Band are requested to contribute a total of $1,870.
